NAFTA Certificate of Origin (Form 434)
US Customs: Personal Effects & Household Goods (Form 3299)
Supplemental declaration to Form 3299 for unaccompanied and household effects
US Importation of Motor Vehicles (Form 3520)

to/returning from non-US countries
Canada Customs: Export Declaration (Form B13)
NOTE: Please have the B13 stamped by Canada Customs prior to sending it to us.
